Life Insurance (www.mosers.org)

Basic Life Insurance

As a Department of Revenue (DOR) employee, you receive one times your annual salary of Basic Life Insurance coverage (rounded to the next thousand) through the Missouri State Employees’ Retirement System (MOSERS) at no cost.

Optional Life Insurance

You may purchase additional term life insurance with premiums paid through payroll deduction. You may choose the following option:

State Voluntary Life Insurance - Universal Life Insurance

The Missouri State Employees’ Voluntary Life Insurance Commission (MOVLIC), established by the 87th General Assembly, selected the McDaniel Hazley Group, Inc./Allstate to manage this universal life insurance program. You may insure yourself, spouse, children, or grandchildren.

Medical Plans - Missouri Consolidated Health Care Plan (www.mchcp.org)

Copay Plan through First Health

With the Copay Plan through First Health you have the freedom to choose your care from any primary care physician (PCP), specialist or hospital. Deductibles: (Network) Individual-N/A, Family-N/A; (Non-Network) Individual-$500, Family-$1,000.

Health Maintenance Organizations (HMO)

This alternative to the Copay Plan is offered in most regions throughout the State. HMOs provide a comprehensive set of services. Care is received only from providers employed by or contracting with each HMO. HMOs have no pre-existing conditions and no annual deductible, however, there is a $25 co-payment per physician visit. The semi-monthly premium is based on the employee’s county of residence.

Dental and Vision Plans (MCHCP)

You can enroll yourself and/or dependents in a dental and/or vision plan as a new employee (first eligible) or during the annual open enrollment period. You must be enrolled in the plan to enroll dependents. Once enrolled in the plan, you cannot terminate coverage during the year unless you are also terminating medical coverage, terminating employment, or retiring. Both the dental and vision plan are available statewide.

Pharmacy Plan (www.express-scripts.com)

A pharmacy plan is available to state employees at no additional charge to the employee. You are automatically enrolled in the pharmacy plan when you enroll in a medical plan. Co-payments will be required when you purchase pharmaceutical drugs of generic formulary, brand formulary, and non-formulary. A mail order benefit is available and may be used to purchase up to a 90-day supply of maintenance medication for two and a half co-payment amounts.

Employee Assistance Program (EAP) (www.mchcp.org/se_member/se_eap.htm)

You may participate in the Employee Assistance Program made available through the Missouri Consolidated Health Care Plan by calling Magellan Health Services at 800-808-2261.

The Employee Assistance Program entitles an employee and any member of the employee's household six counseling sessions per episode per calendar year. The Employee Assistance Program may be utilized by an employee experiencing personal areas of concern such as:

Retirement Plan (www.mosers.org)

You may participate in the retirement plan administered by MOSERS if you are employed in a benefit eligible position that requires at least 1,000 work hours per year. The State makes monthly contributions which cover the full cost of the plan. The amount of each employee’s benefit is determined by age at retirement, months of creditable state service, salary, and payment schedule.

Pay Periods

There are two pay cycles per month. The first pay cycle goes from the first through the 15th of each month. The second pay cycle goes from the 16th through the end of the month.

Payday for the first pay cycle is on the last working day of the month. Payday for the second pay cycle is on the 15th of the following month.

If the 15th falls on a Saturday, you will receive pay on the preceding Friday. If the 15th falls on the Sunday, you will receive pay on the following Monday.

Annual Leave, Sick Leave, and Holidays

As a state employee, you earn five (5) hours of annual leave for each semi-month of service in which you are in pay status for 80 hours or more. If you have 10 years of state employment, you earn six (6) hours of annual leave per semi-month. If you have 15 years of state employment, you earn seven (7) hours per semi-month. You also earn five (5) hours of sick leave for each semi-month of service in which you are in pay status 80 hours or more. The amount of sick leave does not increase with length of service. There are 12 paid holidays each year.

As a state employee your pay must be direct deposited into your bank account each pay period. The State of Missouri can deposit your pay to any account at any bank or branch that accepts direct deposits.

Cafeteria Plan (www.mocafe.com)

You may elect to redirect pre-tax dollars into a trust account from which you can later be reimbursed for eligible expenses. Eligible expenses include state-sponsored medical, adult and child care expenses, and medical expenses not covered by insurance. Participation in the Cafeteria Plan lowers the gross taxable income reported to Internal Revenue Service by the Department of Revenue.

Deferred Compensation (http://mo.csplans.com)

You are eligible to participate in the State of Missouri Deferred Compensation Plan through CitiStreet. Employees may defer a portion of their salary before state and federal income taxes are withheld. The minimum deferral is $12.50 per pay period and the maximum is 100 percent of your monthly salary, not to exceed $15,500/annually, whichever is less. Each employee designates where his or her funds are to be invested by selecting from various options, including money market funds, fixed rate funds, growth funds, etc. For any state employee who defers at least $25 per month and has at least 12 consecutive months of service, the State of Missouri will match the first $25 each month that is deferred.

Missouri's 529 College Savings Plan - MOST (www.missourimost.org)

Missouri now has a “win-win” program that allows Missouri taxpayers to save on their state income taxes and save for their children’s future at the same time. The MOST program was launched in November 1999 to encourage Missourians to save for post-high school education while enjoying significant federal and state tax benefits.

Employment Verification (www.theworknumber.com)

As of November 18, 1998, Human Resource Services and Development stopped processing written requests for verifications. The ‘Work Number for Everyone’ is an automated employment verification service that allows you to have your employment and salary verified within a matter of minutes. No one can access your salary information without your permission. You must provide a verifier with the State of Missouri Company Digit Authorization Code if you want your salary information released. The State of Missouri company code is 10106.
