If a person receives an Insurance Verification Notice, the person must respond to the notice or a driver license suspension will be imposed. The following questions and answers were developed to respond to possible reasons why the person received the Insurance Verification Notice. Please review the questions and answers below.

What do I do if I currently have automobile insurance and received an Insurance Verification Notice?
Check the Vehicle Identification Number (VIN) on your automobile (it is usually located on the driver’s side dashboard at the base of the windshield). Compare the VIN on your automobile to the VIN recorded on your vehicle registration and insurance policy.
If you check the information and everything is correct, you must complete the Insurance Verification Form and send it to:

If the VIN on the vehicle does not match the VIN on the Insurance Verification Notice, please indicate the correct VIN on the Insurance Verification Form along with your insurance information. Send the form to the address or fax number shown above.
What do I do if I have a seasonal vehicle and only insure it when it is in use (Examples: recreational vehicle, historical vehicle, motorcycles, etc.)?
You must complete the Insurance Verification Form and explain that your vehicle is a seasonal vehicle. The explanation must describe the beginning and ending dates and reason for seasonal use of the vehicle.
How do I complete the Insurance Verification Form if I no longer own the vehicle?
Indicate on the Insurance Verification Form that you no longer own the vehicle and the date that the vehicle was sold.
How do I complete the Insurance Verification Form if my vehicle is not operable?
Indicate on the Insurance Verification Form that your vehicle is not operable and the reason why it is not operable.
What will happen if I do not have automobile insurance?
