ATTENTION: Due to unforeseen circumstances, the mailing of the monthly withholding tax voucher booklets will be delayed.  You may access a blank Form 941 (Employers Return of Income Taxes Withheld) at http://dor.mo.gov/forms/MO-941_2011.pdf or you may file your Form 941 electronically at http://dor.mo.gov/business/payonline.php .
