Documents Required to Obtain A Missouri Driver or Nondriver License or Instruction Permit

Applicants must present one or more documents in each of the following categories. Additional documents may be required.

NAME, DATE OF BIRTH, PLACE OF BIRTH (U.S. Citizen)

If you were born in Missouri and need to obtain a certified copy of your birth certificate, you may contact the Missouri Department of Health & Senior Services for assistance.

NAME, DATE OF BIRTH, PLACE OF BIRTH (Non-U.S. Citizen)

SOCIAL SECURITY NUMBER (ALL Applicants)

MISSOURI RESIDENTIAL ADDRESS (ALL Applicants)

All applicants should show residence with any recent document from this list:

NOTE: A Post Office Box is not acceptable as a residential address. You must show proof of residential address and alternate mailing address (if applicable). Current addresses are important! Your license will be mailed to the address provided.

Residential address is the location at which a person has been physically present and regards as home. A residential address is a person's true, fixed, principal, and permanent home, to which a person intends to return and remain, even though currently residing elsewhere. Applicants requesting to use a mailing address different from their residential address must also submit a document from the acceptable residential address documents list to verify their mailing address.

RENEWAL APPLICANTS:

Refer to the information below (or your renewal postcard) for documents required at the time of renewal.

Non-Commercial Driver License (U.S. Citizen): Non-commercial driver license, permit, or nondriver license applicants who previously provided one of the documents for proof of name, date of birth, and place of birth will not be required to present these documents at the time of renewal unless a change has been made to this information. Refer to the Driver License Renewal Postcard (Form 4549).

Non-Commercial Driver License (Non-U.S. Citizen) and all Commercial Driver License applicants: Commercial driver license and commercial instruction permit applicants are required to present their documents for proof of name, date of birth, and place of birth at the time of renewal. Medical certification documents may also be required. Non-U.S. citizens are required to present all documents at the time of renewal. Refer to the Driver License Renewal Postcard (Form 4549A).

NOTE: Renewal postcards are not issued for instruction permits.

Exemption for Applicants Age 65 or Older

Applicants age 65 or older renewing a non-commercial driver or nondriver license or instruction permit are exempt from presenting documents for place of birth. If your Missouri driver or nondriver license or instruction permit is expired more than 6 months (184 days), you do not qualify for this exemption. The exemption also does not pertain to those obtaining a new permit/driver license or nondriver license.

Exemption for Applicants With A 15-year Missouri Driving History

If you are a Missourian who has held a driver license or nondriver license for 15 years or more, you are eligible to obtain a special one-year non-commercial driver or nondriver license. A one-year license may not be renewed, but does provide a full year for you to obtain the documents required to verify name, date of birth, and place of birth.

Special Considerations for Exceptional Circumstances

NOTE: Applicants age 65 or older renewing a non-commercial driver or nondriver license or instruction permit are exempt from presenting documents for place of birth. If your Missouri driver or nondriver license or instruction permit is expired more than 6 months (184 days), you do not qualify for this exemption. The exemption also does not pertain to those obtaining a new instruction permit, driver license, or nondriver license.

In exceptional circumstances, U.S. citizens who wish to obtain a new Missouri non-commercial instruction permit, driver license, or nondriver license, and are unable to obtain a government-issued U.S. Birth Certificate, may provide the following:
