Bill of Sale/Even-Trade Bill of Sale

This document is to be signed either by the seller alone, or by both seller and purchaser. It describes the vehicle sold or evenly traded (year, make, and vehicle identification number), the date of the sale/trade and the purchase (selling) price or trade value.

General Affidavit Form

This document combines six common affidavits in one form.  Following are the six different affidavit types. Note that some require notarization of the applicant's signature and some do not:

1. Duplicate Title (notary required!): Must be completed when applying for a duplicate title when the title application is NOT notarized. The reason the duplicate is needed (original lost or mutilated, for example) must be stated in the affidavit, and if the original title is mutilated, the mutilated title must accompany the application.
2. Non-Use (notary required!): Must be completed when applicant asks to license a vehicle that has not been operated on the highways, and applicant wishes to have registration fee prorated. Submitting an affidavit of non-use does NOT exempt the applicant from applicable renewal penalties.
3. Gift (notary not required): Must be completed when a vehicle is given to another individual as a gift.
4. Residency (notary not required): Must be completed when the owner of the vehicle moves to Missouri and applies for a title on a vehicle as a resident of another state. Applicant must enter the date he/she established residency in Missouri, and this date will determine the month of expiration for the plates.
5. Vehicle Out-of-State (notary not required): Must be completed and signed by the owner of the vehicle when he/she is renewing the license plate while the vehicle is out-of-state (and therefore, has not been safety-inspected). In addition, the applicant must state that the vehicle has not been in Missouri during the 60 days preceding the application.
6. Other (notary not required): May be completed by the applicant when a transaction requires any type of affidavit. In these cases, the form is used to outline the particular circumstances surrounding the transaction.

Odometer Disclosure Form

This form is used to disclose the mileage of the vehicle only when the vehicle's certificate of title does not have a space in the assignment area of the title in which to disclose the mileage or for the purchaser to acknowledge the disclosure. Only one seller and one purchaser need to print and sign their names on this form.

This form is most often used when a vehicle has a Missouri title that was issued prior to August 28, 1989, or when a vehicle has an out of state title that does not have a space to make the odometer disclosure, as required by law.
