Passage of Senate Bill 106 during the 2023 legislative session, required the Missouri Department of Revenue to place the Department of Social Services eligibility review forms, on the Department of Revenue’s website, in order to provide quick and convenient access.

To apply for benefits visit:

• MoHealth: My Healthcare Benefit (mo.gov)
  This healthcare coverage is different from Medicare and it can help with benefits not normally covered through that program, like nursing home care and personal care services.
• Child Care: IM-1CC (mo.gov)
  Finding quality, affordable childcare can be hard, but we may be able to help. Our Child Care Subsidy Program helps eligible families pay for the childcare that fits their schedule.
• SNAP: FS-1-SNAP (mo.gov)
  The Supplemental Nutrition Assistance Program (SNAP) offers a monthly benefit to help those in need buy food and food products.
• TANF: IM-1TALF (mo.gov)
  Temporary Assistance (TA) is a monthly cash benefit to help families in need cover costs for their children, like clothing, utilities, and other services.
